Job summary

One Home Health is seeking a compassionate, motivated Physical Therapist Assistant (PTA) to join our home health team serving Porter County, Indiana and surrounding communities.

As a PTA, you’ll work one-on-one with patients in their homes to improve mobility, restore function, and support independence. This part-time or PRN role offers flexible scheduling, competitive pay, paid time off, and strong clinical leadership in a collaborative team.

Qualifications

• Active Indiana PTA license.
• Graduate of an accredited PTA program recognized by the APTA.
• Minimum six months PTA experience.
• Current CPR/BLS certification.
• Valid driver’s license and transportation.
• Ability to travel throughout Porter County and surrounding areas.

Responsibilities

• Deliver physical therapy treatments and education under the direction of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance, and functional independence.
• Observe and report progress and changes to the supervising PT.
• Complete clinical documentation accurately and on time.
• Educate patients and caregivers on exercises, safety, and home programs.
• Collaborate with therapists, nurses, physicians, and other health professionals.
• Participate in care planning and team meetings as needed.
• Follow safety, infection prevention, and confidentiality standards.
• Maintain compliance with agency policies and state and federal regulations.

Pay: $45.00 - $55.00 per hour

About the Role

As a Home Health PTA, you'll work closely with patients in their homes, helping them improve mobility, increase strength, reduce fall risk, and achieve their rehabilitation goals. You'll be supported by an experienced team of Physical Therapists, nurses, and healthcare professionals dedicated to delivering exceptional patient-centered care.

This role is ideal for a PTA who enjoys working independently while making a lasting impact on patients' quality of life.

Key Responsibilities

• Deliver physical therapy treatments and patient education under the direction of a licensed Physical Therapist and according to the established plan of care.
• Help patients improve mobility, strength, balance, endurance, and functional independence.
• Observe and report patient progress, responses to treatment, and any changes in condition to the supervising Physical Therapist.
• Complete all clinical documentation accurately and within agency timelines.
• Educate patients and caregivers on therapeutic exercises, fall prevention strategies, safety techniques, and home exercise programs.
• Collaborate with Physical Therapists, nurses, physicians, and other healthcare professionals to coordinate care.
• Participate in care planning discussions and interdisciplinary team meetings as needed.
• Follow all safety, infection prevention, and confidentiality standards.
• Maintain compliance with agency policies and state and federal regulations.

Qualifications Required

• Active Indiana Physical Therapist Assistant (PTA) license.
• Graduate of an accredited Physical Therapist Assistant program recognized by the APTA.
• Minimum of six (6) months of PTA experience.
• Current CPR/BLS certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Strong communication, documentation, and interpersonal skills.
• Ability to travel throughout Porter County and surrounding service areas.
• Ability to work independently while providing exceptional patient care.

Preferred

• Home health experience preferred.
• Experience working with geriatric and rehabilitation patients.
• Knowledge of Medicare home health documentation requirements.
• Experience with Axxess software.
• Bilingual candidates are encouraged to apply.
