What You'll Do
As a Physical Therapist Assistant, you will work under the direction of a licensed Physical Therapist to deliver exceptional home health services and support positive patient outcomes.
Key Responsibilities
- Provide physical therapy treatments and patient education in the home according to the physician-approved Plan of Care.
- Complete patient visits and documentation accurately, timely, and in compliance with agency standards.
- Monitor patient progress and promptly communicate any changes in condition, treatment response, or scheduling needs to the supervising therapist or nurse.
- Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team to coordinate patient care.
- Participate in care conferences and contribute to patient care planning as appropriate.
- Adhere to all safety protocols, infection prevention procedures, and proper equipment-use guidelines.
- Protect patient privacy and maintain strict confidentiality in accordance with HIPAA and agency policies.
- Ensure compliance with agency policies and all applicable state, federal, and professional regulations.
- Participate in ongoing education, staff meetings, and quality improvement initiatives.
- Perform other duties as assigned to support patient care and organizational goals.
Required Qualifications
- Current and unrestricted Physical Therapist Assistant license in the state of Indiana.
- Graduate of an accredited Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA).
- Minimum of six (6) months of experience as a Physical Therapist Assistant.
- Strong verbal, written, and interpersonal communication skills.
- Reliable transportation with a valid driver's license, current automobile insurance, and a satisfactory driving record.
