Responsibilities

Deliver outpatient rehabilitation services in patients' homes or independent living facilities, evaluating and treating patients to enhance independence under the supervision of a Licensed Physical Therapist. Responsibilities include:

• Participating in patient care and team meetings.
• Adhering to schedules, documentation, and billing requirements.
• Collaborating with the rehab team to develop and implement patient programs.
• Assisting Physical Therapists during tests and evaluations.
• Instructing patients and caregivers in therapy techniques.
• Maintaining accurate documentation and equipment.
• Providing patient-centered care to ensure the highest quality service.
• Implementing individualized treatment plans, including therapeutic exercises, manual therapy, airway techniques, and more.

Qualifications

• Valid license in the state(s) of practice or proof of license eligibility.
• Reliable transportation to patient residences.
