Job summary

VitalCaring Group in Port Saint Lucie, FL is seeking a Physical Therapy Assistant (PTA) for home health. You will provide direct therapy services in patients' homes, following licensed PT plans, and help regain mobility and independence.

This field-based role offers one-on-one care, flexible schedules, and strong teamwork with therapists, nurses, and care staff. Applicants should have PTA education, state license, CPR, and a driving role; home health experience is a plus.

Qualifications

• Graduate of an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or certification as required by state law.
• 1+ year of experience preferred (home health experience a plus).
• Current CPR certification and valid driver's license.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
• Observe and report patient progress, response to treatment, and any changes in condition.
• Maintain accurate, timely documentation in the EMR and communicate with the care team.

What You’ll Do

Deliver High-Quality Patient Care

• Provide direct physical therapy services under the direction of a licensed Physical Therapist
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility
• Support patients with transfers, strengthening, and activities of daily living
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate
• Educate patients and caregivers on exercises, safety, and use of assistive devices

Monitor Progress & Communicate

• Observe and report patient progress, response to treatment, and any changes in condition
• Maintain accurate, timely documentation in the EMR
• Communicate effectively with the Physical Therapist and interdisciplinary care team
• Participate in case conferences and care coordination efforts
• Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care
• Participate in team meetings, in-services, and ongoing training
• Uphold clinical standards and contribute to quality improvement efforts
