Job summary

VitalCaring Group is seeking a Physical Therapy Assistant (PTA) for home health. You will deliver hands-on care in patients' homes, following PT plans to improve mobility and independence while building strong patient relationships.

The role emphasizes autonomy with PT team support, ongoing training, and flexible field-based work. Requires PTA qualification and current CPR; prior home health experience is a plus.

Qualifications

• Graduate of an accredited PTA program.
• Current PTA license or certification where required by state law.
• 1+ year of experience preferred, home health a plus.
• Current CPR certification.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed PT.
• Follow individualized plans of care to reduce pain and restore function.
• Assist with therapeutic exercises, gait training, balance, and mobility.
• Support transfers, strengthening, and activities of daily living.
• Utilize modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety, and device use.

What You’ll Do

Deliver High‑Quality Patient Care

• Provide direct physical therapy services under the direction of a licensed Physical Therapist
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility
• Support patients with transfers, strengthening, and activities of daily living
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate
• Educate patients and caregivers on exercises, safety, and use of assistive devices

Monitor Progress & Communicate

• Observe and report patient progress, response to treatment, and any changes in condition
• Maintain accurate, timely documentation in the EMR
• Communicate effectively with the Physical Therapist and interdisciplinary care team
• Participate in case conferences and care coordination efforts
• Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care
• Participate in team meetings, in‑services, and ongoing training
• Uphold clinical standards and contribute to quality improvement efforts

What You Bring

• Graduate of an accredited Physical Therapy Assistant (PTA) program
• Current PTA license or certification as required by state law

• Valid driver’s license, reliable transportation, and auto insurance
