Job summary

VitalCaring Group is seeking a dedicated Physical Therapy Assistant to provide in-home care, working one-on-one with patients to improve strength, mobility and independence. You will deliver therapy under a licensed PT, following individualized plans of care in a home setting.

The role requires PTA certification, CPR, and a valid driver’s license, with a focus on compassionate, patient-centered care and collaboration with nurses and therapists. Join a supportive team and grow professionally.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current PTA license or state certification is required by law.
• CPR certification is required or current.
• Valid driver’s license and reliable transportation for home visits.

Responsibilities

• Provide direct physical therapy services under a licensed Physical Therapist.
• Follow individualized plans of care to improve mobility and function.
• Assist with therapeutic exercises, gait training, balance and daily activities.
• Support transfers, strengthening and activities of daily living.
• Use modalities like heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety and devices.

What You’ll Do

Deliver High‑Quality Patient Care

• Provide direct physical therapy services under the direction of a licensed Physical Therapist
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility
• Support patients with transfers, strengthening, and activities of daily living
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate
• Educate patients and caregivers on exercises, safety, and use of assistive devices

Monitor Progress & Communicate

• Observe and report patient progress, response to treatment, and any changes in condition
• Maintain accurate, timely documentation in the EMR
• Communicate effectively with the Physical Therapist and interdisciplinary care team
• Participate in case conferences and care coordination efforts
• Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care
• Participate in team meetings, in‑services, and ongoing training
• Uphold clinical standards and contribute to quality improvement efforts

What You Bring

• Graduate of an accredited Physical Therapy Assistant (PTA) program
• Current PTA license or certification as required by state law
• 1+ year of experience preferred (home health experience a plus)
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
