Job summary

VitalCaring is seeking a dedicated Physical Therapy Assistant (PTA) to provide one-on-one, in-home therapy across our field-based program. You will help patients regain strength, mobility, and independence in their own homes, working under a licensed Physical Therapist.

This role emphasizes hands-on patient care, collaborative teamwork with nurses and therapists, and ongoing growth opportunities. A PTA license and CPR certification are required, with preference for home health experience.

Qualifications

• Minimum 1 year in home health or similar patient-facing setting is preferred.
• Licensed Physical Therapy Assistant (PTA) in the state where you practice.
• CPR certification current and in good standing.

Responsibilities

• Deliver direct physical therapy services under licensed PT supervision.
• Follow individualized care plans to improve mobility and function.
• Assist patients with therapeutic exercises, gait training, balance and ADLs.
• Document progress and communicate with PT and care team.
• Educate patients and caregivers on exercises and safety.

What You’ll Do

Deliver High-Quality Patient Care

• Provide direct physical therapy services under the direction of a licensed Physical Therapist
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility
• Support patients with transfers, strengthening, and activities of daily living
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate
• Educate patients and caregivers on exercises, safety, and use of assistive devices

Monitor Progress & Communicate

• Observe and report patient progress, response to treatment, and any changes in condition
• Maintain accurate, timely documentation in the EMR
• Communicate effectively with the Physical Therapist and interdisciplinary care team
• Participate in case conferences and care coordination efforts
• Work closely with nurses, therapists, and other healthcare professionals to ensure coordinated care
• Participate in team meetings, in-services, and ongoing training
• Uphold clinical standards and contribute to quality improvement efforts

What You Bring

• Graduate of an accredited Physical Therapy Assistant (PTA) program
• Current PTA license or certification as required by state law
• 1+ year of experience preferred (home health experience a plus)
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
