Job summary

One Home Health in Chicago is seeking a compassionate Physical Therapist Assistant to join our home health team serving Downtown Chicago and North Side neighborhoods.

You will work in patients' homes, delivering PT services under licensed PT supervision, focusing on mobility, independence, and rehabilitation goals. We offer flexible scheduling, PTO, paid sick time, competitive pay, and strong clinical leadership in a supportive, interdisciplinary team.

Qualifications

• Active Illinois PTA license.
• Graduate of an accredited PTA program recognized by the APTA.
• Minimum six months PTA experience.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Strong communication and documentation skills.
• Ability to travel throughout Downtown Chicago service area.
• Ability to work independently while delivering patient care.

Responsibilities

• Deliver PT treatments and patient education under the direction of a licensed Physical Therapist.
• Help patients improve mobility, strength, balance, and independence.
• Observe and report progress to supervising PT.
• Complete clinical documentation within agency timelines.
• Educate patients and caregivers on exercises and safety.
• Coordinate care with the interdisciplinary team.
• Participate in care planning and team meetings.
• Follow safety, infection prevention, and confidentiality standards.
• Maintain compliance with agency policies and state/federal regulations.

Compensation & Schedule

Pay: $45.00 - $55.00 per hour

About the Role

As a Home Health PTA, you'll work closely with patients in their homes, helping them improve mobility, regain independence, and achieve their rehabilitation goals. You'll be supported by an experienced team of Physical Therapists, nurses, and healthcare professionals who share a commitment to delivering exceptional patient care.

Key Responsibilities

• Deliver physical therapy treatments and patient education under the direction of a licensed Physical Therapist and according to the established plan of care.
• Help patients improve mobility, strength, balance, endurance, and functional independence.
• Observe and report patient progress, responses to treatment, and any changes in condition to the supervising Physical Therapist.
• Complete all clinical documentation accurately and within agency timelines.
• Educate patients and caregivers on therapeutic exercises, fall prevention, safety techniques, and home exercise programs.
• Collaborate with Physical Therapists, nurses, physicians, and other healthcare professionals to coordinate care.
• Participate in care planning discussions and interdisciplinary team meetings as needed.
• Follow all safety, infection prevention, and confidentiality standards.
• Maintain compliance with agency policies and state and federal regulations.

Qualifications

Required

• Active Illinois Physical Therapist Assistant (PTA) license.

Preferred

• Home health experience.
• Experience working with geriatric and rehabilitation patients.
• Knowledge of Medicare home health documentation requirements.
• Experience with Axxess software.
• Bilingual candidates are encouraged to apply.
