Job summary

Rochester Restorative Care Center is seeking a dedicated Physical Therapist (PT) to evaluate and restore mobility for residents. You will plan and implement individualized therapy, collaborating with an interdisciplinary team to ensure patient-centered care and continuous improvement in function and independence.

The role emphasizes hands-on treatments, teaching safe mobility, and updating plans as progress is tracked.

Qualifications

  • Current Minnesota PT license required.
  • Graduate of an accredited physical therapy program.
  • Strong clinical skills with rehabilitation techniques.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Prior experience in skilled nursing/long-term care preferred.

Responsibilities

  • Evaluate patients’ physical abilities, needs, and goals.
  • Develop individualized treatment plans in line with physician orders.
  • Provide PT treatments including therapeutic exercise, gait, balance and mobility work.
  • Train patients, families, and caregivers on safe mobility and home exercises.
  • Monitor and document patient progress; adjust plans as needed.
  • Collaborate with physicians, nursing and other therapy professionals for coordinated care.
  • Maintain compliance with federal, state and professional standards.
  • Supervise and guide PTAs as appropriate.

Physical Therapist (PT)
Position Summary

The Physical Therapist (PT) is responsible for evaluating, planning, and implementing physical therapy services to restore function, improve mobility, relieve pain, and prevent or limit permanent disability. The PT collaborates with the interdisciplinary team to ensure individualized, resident-centered care that promotes health and independence.

Key Responsibilities
  • Evaluate patients’ physical abilities, needs, and goals through assessments and screenings
  • Develop individualized treatment plans in accordance with physician orders and clinical best practices
  • Provide direct physical therapy treatments including therapeutic exercise, gait training, balance, and mobility interventions
  • Train patients, families, and caregivers on safe mobility, home exercise programs, and use of assistive devices
  • Monitor and document patient progress, updating treatment plans as needed to maximize outcomes
  • Collaborate with physicians, nursing, and other therapy professionals to ensure coordinated care
  • Maintain compliance with federal, state, and professional practice standards
  • Supervise and guide Physical Therapy Assistants (PTAs) as appropriate

  • Current and active Physical Therapist (PT) license in Minnesota
  • Graduate of an accredited physical therapy program
