  • Monitor and document patient progress, updating treatment plans as needed to maximize outcomes.
  • Collaborate with physicians, nursing, and other therapy professionals to ensure coordinated care.
  • Maintain compliance with federal, state, and professional practice standards.
  • Supervise and guide Physical Therapy Assistants (PTAs) as appropriate.

Physical Therapist (PT)
Position Summary:

The Physical Therapist (PT) is responsible for evaluating, planning, and implementing physical therapy services to restore function, improve mobility, relieve pain, and prevent or limit permanent disability. The PT collaborates with the interdisciplinary team to ensure individualized, resident-centered care that promotes health and independence.

Key Responsibilities:
  • Evaluate patients’ physical abilities, needs, and goals through assessments and screenings
  • Develop individualized treatment plans in accordance with physician orders and clinical best practices
