Provide pediatric physical therapy services to children from birth through age 21 in home and community settings under the direction of a licensed Physical Therapist. Support treatment plans focused on mobility, strength, balance, coordination, gross motor development, positioning, and functional movement. Help children build safety, independence, and meaningful progress through individualized therapy interventions. Partner with families and caregivers to promote carryover between visits. Collaborate with physical therapists, occupational therapy, speech therapy, nursing, and care coordination teams. Complete timely documentation using electronic charting tools.
