Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapy Assistant (PTA) on a PRN basis in a Skilled Nursing Facility setting. Flexible, as-needed shifts with an hourly rate based on experience are offered.

The role involves implementing PT plans, leading patients through therapies, educating on safe techniques, and maintaining meticulous records. Applicants should hold an accredited PTA degree with a valid PTA license and CPR certification preferred.

Qualifications

• Associate degree from an accredited PTA program.
• Valid state PTA license.
• Rehab/clinical experience preferred (new grads welcome).
• Strong communication and interpersonal skills.
• Commitment to high-quality, patient-centered care.
• CPR certification preferred.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
• Maintain accurate, compliant therapy records.
• Collaborate with the care team for coordinated treatment.
• Uphold ethical, professional standards in all interactions.
