We are seeking a dedicated Physical Therapy Assistant for our In-House Skilled Nursing Facility. This Per Diem position involves assisting in planning, organizing, developing, and directing physical therapy services to ensure high-quality resident care.

Job Responsibilities:

1. Provide physical therapy assistant services following established standards, procedures, and productivity goals.
2. Ensure proper documentation and billing in accordance with company policies.
3. Evaluate residents to develop and implement treatment plans with measurable goals.
4. Conduct screenings and specialized evaluations as needed.
5. Communicate findings and treatment plans effectively with residents, families, and team members.
6. Maintain compliance with CMS documentation requirements in NetHealth EMR systems.

Qualifications:

• Associate's Degree in Physical Therapy from an accredited program.
