Job summary

Lifepoint Rehabilitation, part of Lifepoint Health, invites a licensed Physical Therapist to join our team at the Community Stroke and Rehab Center in Crown Point, Indiana. The role focuses on compassionate patient care within an adult medical rehabilitation program.

You will assess needs, plan interventions, and collaborate with nursing and therapy colleagues to maintain high standards of practice. Bachelor's in PT, current Indiana licensure, and BLS/CPR are required; inpatient rehab experience

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current licensure as a Physical Therapist in Indiana.
• Current BLS/CPR certification.

Responsibilities

• Assumes responsibility for care of patients under supervision of Physical Therapist and Therapy staff.
• Assesses patient needs and initiates appropriate physical therapy interventions.
• Collaborates with the patient care team to maintain standards of professional practice.
• Performs assessments, reassessments, and documentation including QI scoring.
• Assess and manage pain to support participation in rehabilitation.
