Job summary

First Senior Home Health is seeking a Licensed Physical Therapist Assistant to provide in-home physical therapy under supervision. This role requires Florida licensure, an approved PTA education program, and successful completion of the national PTA exam.

The PTA will participate in treatment planning, documentation, and collaboration with the PT, RN Case Management, and Nursing Supervisor to support patient rehabilitation goals at home.

Qualifications

  • Licensed, registered or certified as a Physical Therapist Assistant in Florida.
  • Passed a national examination for Physical Therapist Assistants.
  • Must be licensed by the State of Florida as a Physical Therapy Assistant.
  • Must have a criminal background check.
  • Must have current CPR certification; online certification is not accepted.

Duties

  • Assume responsibility for self‑development by continually striving to improve health care knowledge through educational programs, workshops, conferences, professional organization participation, research and reading.
  • Assume other duties deemed appropriate and necessary by the PT, Registered Nurse Case Management, and Nursing Supervisor.
Responsibilities
  • Under the direction of a Registered Physical Therapist, provide patient services delegated by the PT.
  • Under the supervision of the physical therapist, assist in evaluation and development of a rehabilitative plan of care and in periodic re‑evaluation as required.
  • Participate in the preparation of clinical and daily progress notes; all notes are reviewed by the PT and submitted within 48 hours, and prepare periodic summary reports.
  • Perform routine strengthening and therapeutic exercises and assist the patient with the use of self‑help devices.
  • Instruct the patient and family, assist in teaching exercise programs.
  • Identify possible ineffective drug therapy, adverse reactions, significant side effects, drug allergies, contra‑indicated medication and report to the PT.
  • Observe, record activities and findings in the clinical record, and report to the PT and physician the patient’s reaction to treatment and any changes in condition.
  • Consult with PT regarding home visit outcomes and report physical findings in written form.
  • Assist in evaluation and development of the rehabilitative plan of care and periodic re‑evaluation, as indicated.
  • Instruct other health team personnel and family members regarding phases of physical therapy.
  • Attend rehabilitation department staff meetings and case conferences.
  • Participate in discharge activities and assist PT in completing the Physical Therapy Discharge Summary.
  • Confirm, on a weekly basis, scheduling of visits with the Supervisor/Clinical Manager to coordinate Agency visits.
Assists in the Agency Admission Process
  • Obtain a medical history, particularly as it relates to the present condition.
  • Evaluate the patient, significant others, and home situation to determine needed instruction, family assistance and other Agency/community services.
  • Ensure physician orders are appropriate and discuss necessary changes.
Qualifications
  • Licensed, registered or certified as a Physical Therapist Assistant in the state of Florida, or meets one of the following: graduated from a PT Assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association; or, if educated outside the U.S. or trained in the U.S. military, graduated from an education program determined to be substantially equivalent to PT Assistant entry‑level education by a credentials evaluation organization approved by the American Physical Therapy Association or identified at 8 CFR 212.15(e).
  • Passed a national examination for Physical Therapist Assistants.
  • Must be licensed by the State of Florida as a Physical Therapy Assistant.
  • Must have a criminal background check.
  • Must have current CPR certification; online certification is not accepted.
Job Conditions
  • Ability to drive and access patients' homes, which may not be wheelchair accessible.
  • Hearing, eyesight, and physical dexterity sufficient to perform assessment and patient care.
  • May be required to bend, stoop, reach, and move patient weight up to 250 pounds; lift/carry up to 30 pounds.
  • Must be able to effectively communicate in English.
