Job summary

Freedom at Home is seeking compassionate Physical Therapist Assistants (PTAs) to join our Home Health team. This Part-Time / PRN role offers flexible scheduling, paid mileage, and the opportunity to deliver high-quality care in patients' homes.

You will work under licensed PT supervision, implementing treatment plans and educating caregivers, while documenting care and collaborating with a supportive team committed to patient independence and comfort.

Qualifications

• Graduate of an accredited PTA program.
• Current PTA license in the state of practice.
• Current CPR certification.
• Minimum of two years PTA experience preferred.
• Home Health experience is a plus but not required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled physical therapy services under the supervision of a licensed Physical Therapist.
• Carry out treatment plans to improve mobility, strength, balance and function.
• Assist patients with therapeutic exercises, gait and transfer training, and use of devices.
• Monitor progress and communicate changes to the supervising PT.
• Educate patients and caregivers on home exercise programs and safety.
• Complete accurate, timely clinical documentation.
• Participate in team meetings and quality improvement initiatives.
• Collaborate with physicians, nurses and other professionals to coordinate care.
• Promote safe, high-quality care that supports home independence.

Responsibilities

• Provide skilled physical therapy services under the direction and supervision of a licensed Physical Therapist.

• Carry out individualized treatment plans designed to improve patients' mobility, strength, balance, function, and overall quality of life.

• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and use of assistive devices.

• Monitor patient progress and communicate changes to the supervising Physical Therapist.

• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.

• Complete accurate, timely clinical documentation.

• Participate in case conferences, team meetings, and quality improvement initiatives.

• Collaborate with physicians, nurses, therapists, and other healthcare professionals to provide coordinated patient care.

• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes.
