Job summary

PeaceHealth SHMC at Riverbend in Springfield, OR is seeking a Physical Therapist Neuro for Outpatient Therapy for a Part Time, 0.80 FTE, Day position. The salary range is $48.52 – $72.78 per hour, with relocation assistance may be available.

Responsibilities include evaluation, planning, documentation and collaboration with the care team. Qualified candidates hold an Oregon PT license, a degree in physical therapy, and BLS/CPR certification.

Qualifications

  • Physical Therapist degree accredited by CAPTE or equivalent.
  • Licensed to practice in Oregon at hire.
  • Basic Life Support (BLS/CPR) certification required at hire.
  • Neuro physical therapy experience is preferred.

Responsibilities

  • Evaluate patients and establish a treatment plan and goals.
  • Provide patient treatment per plan and adjust as needed.
  • Document accurately and timely per regulations and standards.
  • Collaborate with healthcare professionals to ensure continuous care.
  • Mentor new staff and students; participate in program development.
  • Participate in meetings, education, and quality improvement activities.
  • Perform other duties as assigned.

Job Summary

Responsible for the evaluation of patients, determining plan-of-care and goals of treatment, participating in treatment of patients within the scope of Physical Therapy. May provide oversight to therapy assistants and/or students in following the plan-of-care and treatment of patients appropriate to the age of the patient served; may provide supervision to therapy aides and/or volunteers. Works as part of a patient care team toward the best outcomes for the patient.

What you will do:
  • Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation, and team members.
  • Provides patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate.
  • Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
  • Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.
  • May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students.
  • Recommends and participates in development of new services and programs, quality improvement and monitoring activities.
  • May participate in staff meetings, patient care meetings, educational in-services, caregiver orientation and other professional activities.
  • Performs other duties as assigned.
What you bring:
Education
  • Bachelor's, Master's or Doctorate Degree Required: Physical Therapyaccredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted)
  • Required: Physical Therapist License State of Oregon
  • Required: Upon Hire Basic Life Support through American Heart Association or American Red Cross
  • Preferred: Neuro Physical Therapy experience
