Job description

Overview

Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist ("PT"). Assists in monitoring patient progress through observations and follows up with the supervising therapist. Works under PT supervision as required under NY State Education law.* Follows established physical therapy plan of care, including interventions and patient/caregiver education. Modifies specific treatment procedures in accordance with PT plan of care and PTA scope of practice.

* Documents all care provided and maintains records per VNS Health policy and standards.

* Assists in monitoring patient progress through observation and follows up with Supervising Therapist in accordance with VNSNY policy.

* Complies with scope of practice for PTA, including co-visits with PT on initial visits and keeping PT informed. PT will re-assess every 6 visits or 30 days, whichever comes first.

* Manages assigned caseload; provides services as directed per PT plan of care.

* Participates in VNS Health meetings, including interdisciplinary team meetings, in-service and training programs, rehab services meetings, and caseload reviews.

* Travels to patients' homes and/or other facilities with varying environments using approved transportation options to deliver care.

* Assists patients with therapeutic interventions and activities of daily living, which may include positioning, moving, transferring, and lifting patients of varying sizes and conditions, with or without assistance. Instructs and demonstrates to family members and paraprofessionals. Assists with durable medical equipment, including wheelchairs, lifts, ambulatory devices, splints, orthoses, prostheses, and exercise equipment.

* Provides care in accordance with agency policies and professional standards, which may require standing, stooping, sitting, crouching, twisting, reaching, bending, and stretching.

* Transports and utilizes technology devices, infection control materials, and supplies using designated carrying cases.

* Participates in special projects and performs other duties as assigned.

Qualifications

Licenses and Certifications:

• New York State Certification as Physical Therapist Assistant required
• Valid driver's license may be required based on operational needs

Education:

• Associate's Degree as a Physical Therapist Assistant from a NYS-approved program or equivalent, required

Work Experience:

• Minimum of two years of clinical patient care experience, required
• Bilingual skills and basic PC skills may be required, depending on operational needs

Compensation

$30.71 - $38.41 Hourly
