Job summary

Bergen Care Home Health is seeking a Physical Therapy Assistant to join our home care team in Newark, NJ. The PTA will manage resident rehabilitation within accepted practice standards and work as part of an interdisciplinary team to balance outcomes and cost‑effective care.

Essential duties include evaluating resident needs, creating and adjusting treatment plans, supervising plan execution, documenting care and billing, and supporting marketing efforts.

Qualifications

  • Bachelor's Degree in Physical Therapy required.
  • Current and Active Physical Therapy Assistant license within the state of operations required.
  • Prefer experience/knowledge in older adult rehabilitation.
  • Prefer knowledge of governmental and regulatory guidelines in designated healthcare settings.

Responsibilities

  • Provides and directs resident rehabilitation care within general accepted practice methods and meets center, state, federal, and accrediting standards.
  • Evaluates residents' needs and establishes effective treatment plans.
  • Coordinates supervision of treatment plans with staff and other disciplines.
  • Screens residents according to schedules and practices.
  • Provides feedback to supervisor on clinical and rehabilitation issues.
  • Participates in center-directed processes and services.
  • Manages treatment and non-treatment related activities effectively.
  • Completes clinical documentation to meet all regulatory requirements.
  • Maintains billing records to meet requirements.
  • Assists with marketing efforts as directed.
  • Travel to other facilities to provide services if needed.

Education

Bachelor's Degree in Physical Therapy
Current and Active Physical Therapy Assistant license within the state of operations

The Physical Therapy Assistant's (PTA) overall responsibility is to manage the care of the resident/resident, managing quality of all related outcomes. The PTA participates as an integral part of the interdisciplinary team, manages resources to meet the needs of the resident/resident and related customers balancing outcomes and cost effective and efficient treatment.

Essential Duties and Responsibilities:

  1. Provides and directs resident/resident rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
  2. Evaluates residents/resident needs and establishes effective and efficient treatment plans for the resident/residents assigned.
  3. Manages and coordinates the supervision of treatment plans with all staff and other disciplines as indicated.
  4. Participates in the screening of all residents according to directed schedules and practices.
  5. Provides feedback to the designated supervisor on all clinical and rehabilitation issues.
  6. Participates in all center directed processes and services.
  7. Shows effective management of treatment and non-treatment related activities.
  8. Performs necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
  9. Maintains complete, accurate and timely billing records to meet center, state, federal, and accrediting agency requirements
  10. Assists with marketing efforts as directed.
  11. Participates in in-service training programs as requested 12. May travel to other facilities to provide services

Education:

Bachelor's Degree in Physical Therapy

Professional Experience:

  • Current and Active Physical Therapy Assistant license within the state of operations
  • Prefer-experience/knowledge in older adult rehabilitation.
  • Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings
