Job summary

Ascend Health in Newark, NJ is seeking a Physical Therapy Assistant to provide and direct rehabilitation services in home care. You will evaluate residents, develop and monitor treatment plans, and collaborate with nursing and therapy teammates to ensure high-quality, compliant care.

This role requires an active PTA license in the operating state and preference for experience with older adults and familiarity with regulatory guidelines.

Qualifications

• Current and active Physical Therapy Assistant license in the operating state.
• Prefer experience with older adult rehabilitation.
• Familiarity with governmental and regulatory guidelines in healthcare settings.

Responsibilities

• Provides and directs resident rehabilitation care in line with accepted practices and standards.
• Evaluates residents and creates effective treatment plans.
• Coordinates supervision of treatment plans with staff and other disciplines.
• Screens residents as scheduled and follows directed practices.
• Provides feedback to supervisor on clinical issues.
• Participates in center-directed processes and services.
• Manages treatment and non-treatment activities effectively.
• Completes clinical documentation to meet regulatory requirements.
• Maintains accurate billing records as required.
• Assists with marketing efforts as directed.
• Participates in in-service training; may travel to other facilities.

• Provides and directs resident/resident rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
• Evaluates residents/resident needs and establishes effective and efficient treatment plans for the resident/residents assigned.
• Manages and coordinates the supervision of treatment plans with all staff and other disciplines as indicated.
• Participates in the screening of all residents according to directed schedules and practices.
• Provides feedback to the designated supervisor on all clinical and rehabilitation issues.
• Participates in all center directed processes and services.
• Shows effective management of treatment and non-treatment related activities.
• Performs necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
• Maintains complete, accurate and timely billing records to meet center, state, federal, and accrediting agency requirements
• Assists with marketing efforts as directed.
• Participates in in-service training programs as requested 12. May travel to other facilities to provide services

Education:

• Bachelor's Degree in Physical Therapy

Professional Experience:

• Current and Active Physical Therapy Assistant license within the state of operations
• Prefer-experience/knowledge in older adult rehabilitation.
• Prefer knowledge in governmental and regulatory guidelines in designated healthcare settings
