The PTA will work with interdisciplinary teams, document notes accurately, and support home health therapy including OASIS while maintaining patient safety.

Qualifications

• Graduation from an accredited PTA program.
• Current, unrestricted state license as a PTA.
• Proficiency in assessments, documentation, and policy adherence.
• Preferred: home health therapy experience and OASIS documentation.

Responsibilities

• Performing direct physical therapy according to physician’s plan and therapist guidance.
• Updating patients and caregivers about the care plan.
• Reporting patient observations and progress to supervisors and physicians.
• Relieving pain, restoring function, and ensuring patient safety during treatments.
• Teaching proper use of prosthetic, orthotic, and assistive devices.
• Collaborating on patient care plans and planning discharge procedures.
