Job summary

PureHealth is seeking a licensed Physical Therapy Assistant to deliver therapeutic services under the supervision of a Physical Therapist, adhering to policy, procedure, and state and federal regulations.

You will contribute to patient assessment, implement treatment plans, document daily charges and progress notes, educate patients, coordinate discharge planning, and work with a collaborative care team in a post-acute setting.

Qualifications

• Graduate of an accredited therapy assistant program.
• At least 1 year therapy assistant experience.
• Valid PTA license.
• HIPAA compliance.
• Ability to work in a post-acute setting.

Responsibilities

• Under the direction of the Physical Therapist, assist with customer assessment related to diagnosis.
• Assist in treatment planning and implementation aligned with customer needs.
• Complete and accurate daily charges, progress notes, and reports.
• Monitor and modify therapeutic benefits as needed.
• Educate customers and families; document education provided.
• Coordinate collaboration and discharge planning with the care team.
• Demonstrate competency in clinical skills across units annually.
• Organize schedules to meet department workload and keep supervisor informed.
• Contribute to department operations including quality improvement and budgeting.
• Comply with HIPAA and licensing requirements.
