Job summary

Emblem Home Health & Hospice in Arizona is hiring a Licensed Physical Therapy Assistant (PTA) to provide hands-on care under the supervision of a Physical Therapist. This full-time role offers a flexible schedule across the East Valley, including Mesa, Tempe, Chandler, and Gilbert, empowering you to balance professional and personal life.

You will deliver treatments, guide therapeutic exercises, monitor progress, educate patients on mobility aids, and maintain accurate notes.

Qualifications

• Licensed PTA in Arizona.
• Graduate from a CAPTE-approved PTA program.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Key Responsibilities

• Deliver physical therapy treatments per PT and physician’s plan of care
• Guide patients through therapeutic exercises and mobility training
• Monitor and report patient progress and condition changes
• Educate patients on mobility aids and prosthetic devices
• Maintain accurate clinical notes and participate in team meetings

Requirements

• Licensed Physical Therapy Assistant (PTA) in the State of Arizona
• Graduate of a CAPTE-approved PTA program (passed national exam)
• Current CPR certification
• Valid driver’s license, insurance, and reliable transportation
