Job summary

Home Sweet Home Health Agency is actively hiring a dedicated Physical Therapist Assistant (PTA) to join our growing home health team in Maricopa County. You will deliver skilled, one-on-one therapy in patients’ homes, focusing on rehabilitation and functional mobility.

We offer flexible schedules (per diem, part-time, full-time) with autonomy to manage your own patient load, and a supportive team dedicated to senior care and community-based services.

Qualifications

• Active Arizona PTA license.
• Current CPR certification (AHA).
• Minimum 1 year of clinical experience; home health preferred.
• Strong interest in home health, rehab and senior care.
• Excellent communication and organizational skills.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises and mobility training.
• Support seniors in improving strength, balance and independence.
• Monitor, document, and report patient progress to the supervising PT.
• Educate patients and caregivers on home exercise programs and fall prevention.
• Collaborate with physicians, RNs and the care team.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities

• Provide in-home physical therapy services following the established Plan of Care


• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs


• Support senior care patients in improving strength, balance, and independence


• Monitor, document, and report patient progress to the supervising Physical Therapist


• Educate patients and caregivers on home exercise programs, safety, and fall prevention


• Collaborate with physicians, RNs, and the interdisciplinary care team

Qualifications

• Active Arizona Physical Therapist Assistant (PTA) license


• Current CPR certification (AHA required)


• Minimum 1 year of clinical experience (home health experience preferred)
