We are seeking a licensed Physical Therapist Assistant to join our growing team supporting patients across West Michigan.. This role offers a high level of autonomy, allowing clinicians to practice at the top of their license while delivering care in patients’ homes. Home health physical therapy provides a unique opportunity to help patients regain independence and function in the environment where they live and thrive. Our therapists maintain manageable caseloads, typically seeing 5-7 patients per day, with thoughtfully designed geographic territories that minimize travel and support work-life balance. As part of Corewell Health West, our VNA team is committed to delivering exceptional, patient-centered care across West Michigan.
Provides rehabilitation therapy procedures to patients under the direction and supervision of a physical therapist. Assists with patients/caregiver education.
