Job summary

Beaumont Home Health and Hospice seeks a qualified Physical Therapist Assistant to deliver skilled services in patients' homes under the supervision of a Physical Therapist. You will educate patients and families, ensure safety, and document care per Medicare guidelines.

Qualifications

  • Must hold a current PTA license in the state of employment.
  • Minimum of one year of PTA experience in an acute setting is required.
  • Home health experience is desired.
  • Ability to communicate effectively with patients, families, physicians, and coworkers.

Responsibilities

  • Provide skilled PTA services in patients' homes as per the plan of care.
  • Document patient progress and maintain chart accuracy.
  • Perform PT procedures and assist with rehabilitation goals.
  • Assist with patient/family education on therapies and safety.

The Physical Therapist Assistant educates patients and their family members and ensures safety of the patient. The PTA is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.
