Job summary

Summa Health at Home and Hospice is seeking a dedicated Physical Therapist Assistant to deliver in-home skilled services under a Physical Therapist’s plan of care. You will educate patients and families, ensure safety, and maintain compliant documentation on a regular basis.

The ideal candidate holds an active PTA license in the employment state with at least one year of acute-care PTA experience; home care experience is a plus, and strong communication skills are essential to coordinate with

  • Physical Therapist Assistant with a current license in the state of employment
  • Minimum of one year of PTA experience in an acute setting required
  • Home care experience is desired
  • Ability to communicate effectively with patients, families, physicians, and co-workers
  • Knowledgeable on therapy best practices and documentation requirements

Responsibilities

  • Provide skilled PTA services in patients' homes under the Physical Therapist plan of care
  • Observe, document, and report patient progress and reactions within required timelines
  • Assist in patient education for therapy techniques, safety, and equipment use
  • Collaborate with the Physical Therapist, physicians, and other staff to coordinate care
  • Complete all clinical documentation in line with agency and Medicare guidelines

It includes patient care, preparation time, travel, lab drops, picking up supplies, charting, and communication with the patient’s family, physician, and other healthcare providers.HEALTH QUALIFICATIONSHealth Requirements:Must be able to meet all physical position requirementsEvidence required of a current negative tuberculin skin test or X-ray, or TB testing at the date of orientationThereafter, an annual tuberculin skin test is required unless the employee has documentation of a previous TB testMust be free of physical/medical conditions which would limit or restrict ability to perform essential functionsMay be at risk for exposure to bloodborne pathogensPHYSICAL DEMANDSThe physical demands described here are representative of those required to successfully perform the essential functions of this job.
