Job summary

Total Care Therapy is seeking a compassionate Physical Therapist Assistant (PTA) to join our growing team in Toledo, OH. You will work with a licensed Physical Therapist to deliver high-quality, patient-centered care focused on mobility, strength, and independence for residents.

You will provide skilled treatments, document outcomes in an EMR, educate patients and caregivers, and collaborate with an interdisciplinary team to ensure coordinated care for older adults in assisted living and

Qualifications

• Current State Physical Therapy Assistant (PTA) license.
• Current CPR/BLS certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist and according to the established plan of care.
• Deliver therapeutic exercises, gait training, balance activities, transfer training, and mobility interventions to help residents achieve functional goals.
• Monitor patient progress and communicate changes in condition to the supervising Physical Therapist.
• Accurately complete documentation and treatment notes using an electronic medical record (EMR) system.
• Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies.
• Collaborate with Physical Therapists and interdisciplinary team members to ensure coordinated, patient-centered care.
• Maintain a full-caseload while providing compassionate, high-quality therapy services.
