Job summary

Lotus Village Center for Nursing & Rehabilitation in Sparta, North Carolina, is seeking a licensed Physical Therapist Assistant (PTA) to provide hands-on care and help our residents achieve their therapy goals in a skilled nursing setting.

This full-time role offers a day shift preferred with flexibility, and a $1000 sign-on bonus. You will follow treatment plans, provide therapy, and collaborate with the therapy team, nursing staff, and families to support resident progress.

Qualifications

• Graduate of an accredited PTA program.
• Current state licensure as a PTA.
• LTC/SNF experience is a plus, but not required.
• Ability to lift up to 50 lbs and assist residents.

Responsibilities

• Follow treatment plans as directed by the Physical Therapist.
• Provide one-on-one and group therapy to residents.
• Track daily notes, treatment progress, and billing.
• Assist with discharge planning and patient education.
• Communicate with therapy team, nursing, and families.
• Participate in care meetings and facility initiatives.

Position Details:

• Status: Full-Time & PRN
• Shift: Day shift preferred (flexibility may be available)
• Setting: Long-Term Care / Skilled Nursing Facility

Responsibilities:

• Follow treatment plans as directed by the Physical Therapist
• Provide one-on-one and group therapy to residents
• Track daily notes, treatment progress, and billing
• Assist with discharge planning and patient education
• Communicate with therapy team, nursing, and families
