Job summary

Paradigm at the Oaks in Schulenburg, TX is seeking a Physical Therapy Assistant to join our in-house rehab team delivering person-centered care. The role includes administering prescribed therapies, documenting outcomes, and coordinating with a supervising PT to optimize patient recovery.

New grads are welcome; active Texas PTA license required. In-house rehab, flexible scheduling, and a $5,000 sign-on bonus are offered to attract dedicated professionals.

Qualifications

• Active/valid Texas PTA license is required.
• SNF/long-term care experience preferred.
• Knowledge of Net Health electronic documentation is preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Administer prescribed PT treatments to relieve pain and improve function.
• Provide PT services under supervision of a Physical Therapist.
• Respond to service requests by relaying information to the PT.
• Implement/modify treatment plan under PT supervision.
• Measure and record motor function, strength, and performance.
• Monitor patient response and adjust treatment with IDT collaboration.
• Provide clinical support and education to patients and families.
• Ensure timely, compliant documentation per regulations.
• Attend meetings designated by the Director of Rehab.
