Midtown Oaks Post Acute in Sacramento, CA is seeking a motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a part-time basis. You will provide therapy under the supervision of a licensed Physical Therapist and help patients regain mobility and independence.

Qualifications

• Licensed PTA in California (current)
• Graduated from an accredited PTA program
• Experience in skilled nursing or post-acute care preferred
• Strong clinical, communication, and documentation skills

Responsibilities

• Provide PT treatments under licensed PT supervision
• Assist patients to improve mobility, balance, strength, endurance, independence
• Implement individualized exercise programs
• Document treatments and patient progress
• Collaborate with therapy staff, nursing, physicians, residents, families
• Participate in discharge planning and continuity of care
• Maintain compliance with facility and regulatory standards

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist
• Assist patients in improving mobility, balance, strength, endurance, and functional independence
• Implement individualized treatment plans and therapeutic exercise programs
• Accurately document treatments, patient responses, and progress toward goals
• Collaborate with therapists, nursing staff, physicians, residents, and families to support optimal outcomes
• Participate in discharge planning and continuity of care initiatives
• Maintain compliance with all facility, state, and federal regulations

Qualifications
