Summary

• This position treats patients in a variety of modalities, educate the family/patient, and assures continuity of treatment. Responsible for all necessary documentation as required by accepted medical guidelines.

• This position works under the supervision of the Registered Physical Therapist and is employed by Heartland Long Term Acute Care Hospital.

Duties

• Treats patients using a variety of modalities and procedures under treatment plan formulated by a Registered Physical Therapist.
• Provides patient/family education.
• Documents patients’ response to treatment program, communicates with physicians, Physical Therapist, and other interdisciplinary team members, and inputs charges as appropriate.
• Participates in staff meetings, family conferences and interdisciplinary team meetings.
• Participates in continual process improvement.

Qualifications

• Graduate of an accredited Physical Therapy Assistant Program.
• Licensed in state of Missouri. Heartsaver certification required within 90 days.
