Job summary

Millennium Physician Group is seeking Physical Therapy Assistants to join our Home Health Team serving Charlotte and Collier Counties, Florida. In this PRN per diem role, you will deliver skilled PTA services under the supervision of a Physical Therapist, helping patients improve mobility and independence in their homes.

You will implement individualized treatment plans, including therapeutic exercises, gait training, and assistive device training, while educating patients and families.

Qualifications

• Active Florida PTA license.
• Minimum 6 months hospital, SNF, or rehab experience.
• Excellent communication and critical thinking skills.

As part of our compassionate, high-performing care team, you will provide skilled therapy services under the direction and supervision of a Physical Therapist to help patients improve mobility, function, and independence. You will implement individualized treatment plans, including therapeutic exercises, gait training, and assistive device training, while educating patients and families on techniques that support recovery and daily living. You will collaborate with an interdisciplinary care team, maintain accurate clinical documentation, and adhere to infection control and safety standards to ensure high-quality patient care and optimal outcomes.

This is a Per Diem (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organizational requirements.

What You'll Do

• Patient Care: Provide skilled therapy services under the supervision of a Physical Therapist.

• Infection Control: Adhere to strict infection control guidelines to protect both patients and healthcare workers.

What We're Looking For

• Licensure: Current and valid Physical Therapist Assistant (PTA) license in the state of Florida.
• Experience: Minimum of 6 months in a hospital, skilled nursing facility, or rehab required.
• Skills: Excellent communication, interpersonal, and critical thinking skills.
• Professionalism: Strong work ethic, reliability, and a commitment to providing quality care.
• Transportation: Reliable transportation to visit patients in their homes.
