Responsibilities

• Conduct treatment plans as established by PT under supervision of PT.
• Communicate with PT, rehab team members and/or referring physician regarding patients' treatment program.
• Maintain weekly objective progress notes, following documentation guidelines.
• Maintain accurate daily attendance records.
• Complete daily time logs, modality sheets, Medicare logs, change slips and any other required paperwork.
• Supervise supportive staff in clerical and transporting duties as assigned.
• Follow all established policies and procedures of Indiana Fitness Works.
• Communicate the PT’s designed plan of care to physician; take telephone orders to certify plan of care, co-signed by supervising therapist.

Requirements:

• Graduate from an accredited Physical Therapist Assistant program. The program must be accredited by the American Physical Therapy Association.

• Lift up to 10lbs overhead occasionally
• Lift up to 50lbs from floor to knuckle occasionally
• Lift up to 25lbs from knuckle to shoulder occasionally
• Pivot transfer up to 150lbs occasionally
• Carry up to 50lbs, 200ft frequently
• Push/pull 75lbs, 100ft frequently
• Fine hand manipulation, bilaterally continuously

Typical Working Conditions:

Duties:
