Job summary

Community Health at Home is seeking a licensed Physical Therapist Assistant to provide in-home patient care across Central/Southern Marion and Johnson County, Indianapolis, IN. You will support mobility and functional goals in a patient-centered home-health setting, under the supervision of a PT, collaborating with families and care teams.

The position offers Mon–Fri 8:00 AM–5:00 PM hours, mileage reimbursement (58.5 cents per mile), and competitive wages.

Qualifications

• Must have Associate's degree in Physical Therapist Assistant from an accredited program
• Active PTA license in the state where you practice
• Current CPR/BLS certification required
• One year PTA experience in a clinical setting
• Current driver's license and automobile insurance
• Comply with federal, state and local laws and regulations

Responsibilities

• Assist patients in their homes to regain mobility, strength and independence
• Collaborate with PT, patients, families and other healthcare professionals to deliver patient-centered care
• Support goal achievement and treatment plans with compassionate care

Job Requirements

• Associate’s degree in Physical Therapist Assistant from an accredited program
