Job summary

Storybrook Care and Rehab in Fort Collins, CO is seeking a compassionate Physical Therapy Assistant to join our in-house therapy team at a midsize long-term care facility. This role supports a dedicated clinical staff in a low-staffing-ratio setting, with a strong team culture and experienced administrator.

• Experience in geriatric therapy is preferred.
• Ability to collaborate with therapy team and interdisciplinary team.

Responsibilities

• Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist.
• Communicate with the Physical Therapist any pertinent information concerning patient progress, concerns, and plans.
• Be proficient in therapeutic needs to treat patients based on their presentation and instructions given by the Physical Therapist
• Participate in patient Plan of care meetings if asked by the Director of Rehab.
• Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedures.

Our family owned and operated long term care facilitator is seeking a compassionate and well versed Physical Therapy Assistant for consideration to add to the inhouse therapy team a midsize facility situated in Northern Colorado, amongst Larimer County. This building is home to a diverse group of long-term residents, cared for by seasoned staff with low staffing ratios, who are stewarded by a well-equipped clinical team and Administrator with years of experience in the geriatric field.

The primary purpose of this position is to plan, organize, develop, and direct our facility's Physical Therapy Services in accordance with applicable federal, state, and local standards, guidelines, regulations to assure that the highest degree of quality care can be always maintained.
