Job summary

Millennium Physician Group in Florida is seeking a Physical Therapist to join our Home Health Team serving Sarasota County, visiting patients in their homes and delivering individualized treatment plans to improve mobility and independence.

You will assess patients, implement therapeutic interventions, educate families, collaborate with a care team, and maintain compliant documentation to ensure high-quality outcomes.

Qualifications

• Florida PT license required.
• Minimum of 1 year of clinical PT experience; home health experience preferred.
• Strong clinical and interpersonal skills; patient-focused approach.

Responsibilities

• Patient Assessment: evaluate patients’ physical abilities and needs to create individualized plans.
• Therapeutic Interventions: implement manual therapy, therapeutic exercises, and gait training.
• Patient Education: educate patients and families on home exercise programs and injury prevention.
• Collaboration: work with physicians, nurses, and therapists to develop care plans.
• Documentation: maintain accurate evaluations, treatment plans, progress notes, and discharge summaries.
