Job summary

Millennium Physician Group is seeking a Physical Therapist to join our Home Health Team in Collier County. You will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence in the patients' homes.

This PRN position offers flexible hours based on staffing needs, patient volume, and organizational requirements.

Qualifications

• Florida license to practice physical therapy (PT) in good standing.
• Minimum 1 year of clinical PT experience, preferably home health or related setting.
• Strong clinical assessment, treatment planning, and documentation skills.

Responsibilities

• Assess patients’ physical abilities and develop individualized treatment plans.
• Perform therapeutic interventions including manual therapy, exercise, and gait training.
• Educate patients and families on home programs and prevention strategies.
• Collaborate with physicians, nurses, therapists, and other team members to optimize care.
• Maintain accurate and timely patient documentation and discharge summaries.
• Adhere to HIPAA, Medicare, and regulatory guidelines.
