Millennium Home Care in Florida is seeking a compassionate Physical Therapist to join our in-home care team on a PRN basis, delivering high-quality assessments and individualized treatment plans to patients in their homes. The ideal candidate holds a current Florida PT license, has at least one year of clinical experience (home health preferred), and demonstrates strong communication, professionalism, and reliability.

Transportation to patient visits is required.

Qualifications

• Licensed Physical Therapist in Florida.
• Minimum of 1 year of clinical PT experience, home health preferred.
• Reliable transportation to visit patients at home.

Responsabilités

• Assess patients' physical abilities and develop individualized treatment plans.
• Provide manual therapy, therapeutic exercises, and gait training.
• Assist with functional training and ADL independence.
• Educate patients and families on home exercise programs and body mechanics.
• Collaborate with physicians and nurses to create care plans.
• Maintain accurate documentation of evaluations, plans, and progress.
• Ensure HIPAA and Medicare compliance.

Responsibilities

• Patient Assessment: Conduct comprehensive evaluations of patients’ physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
• Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients’ mobility, strength, balance, and pain management.
• Functional Training: Assist patients in developing compensatory strategies, adaptive techniques, and assistive device training to enhance their independence in activities of daily living.
• Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long-term health and well-being.
• Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards and guidelines, including HIPAA and Medicare regulations.

Requirements

• Licensure: Current and valid Physical Therapist license in the state of Florida.
• Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health or a related setting.
• Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
• Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.
