Job summary

Deer Meadows Rehabilitation and Nursing invites a dedicated Physical Therapist (PT) to join our resident-centered team. You will assess patients needing rehabilitation, develop and administer treatment plans, and work under physician direction to achieve meaningful goals.

Responsibilities include documentation, progress notes, and discharge planning, with collaboration across disciplines to deliver quality, patient-centered care.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care preferred but not required.
• Experience with Case Mix Index and PPS scheduling preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer a treatment program with goals based on patient capacity.
• Set realistic and achievable goals for patients and document progress.
• Document progress notes, recertifications and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Comply with confidentiality, HIPAA and Residents' Rights.

Duties and Responsibilities of the Physical Therapist (PT):

• Responsible for the assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
