Job summary

Deer Meadows Rehabilitation and Nursing is seeking a Physical Therapist (PT) to join our team. You will assess referred patients and provide direct care to evaluate medical conditions and rehabilitation potential in a geriatric population.

Responsibilities include developing individualized treatment programs, documenting progress, and collaborating with an interdisciplinary team to deliver patient-centered care. Licensing in state and accreditation standards are required.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed to practice Physical Therapy in the state of practice.
• Experience in long-term care or geriatric care preferred.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and rehabilitation potential.
• Establish and administer a treatment program with specific goals.
• Set realistic and achievable goals for patients.
• Document and educate patients and families in an appropriate physical therapy method.
• Perform evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Perform all other duties assigned by Director of Rehabilitation.
• Comply with confidentiality, HIPAA and Residents’ Rights.

Duties and Responsibilities of the Physical Therapist (PT):

• Responsible for the assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Requirements/Qualifications of the Physical Therapist (PT):

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
