Job summary

Synchrony Rehab in Indiana is seeking a Physical Therapist to provide assessment, planning and therapeutic interventions in an interdisciplinary setting. The role requires a degree in Physical Therapy and a current state PT license, with CPR certification preferred.

You will conduct patient assessments, develop individualized treatment plans, educate patients and families, and document progress in compliance with facility policies. Collaboration with the team is essential.

Qualifications

• Education: Degree in Physical Therapy from an accredited program.
• Experience: 0-1 years in physical therapy.
• Must have and maintain a current state PT licensure; CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy, and modalities.
• Educate patients and families on exercise techniques, posture, body mechanics, and condition management.
• Maintain accurate documentation of patient progress and treatment plans in compliance with policies.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
