Job summary

Synchrony Rehab in Indiana is seeking a Physical Therapist to provide comprehensive PT services, including assessment, treatment planning, and therapeutic interventions in an interdisciplinary setting.

You will conduct thorough evaluations, develop personalized plans, educate patients and families, document progress, and collaborate with the care team. Licensure required; CPR preferred; 0-1 years experience accepted; on-site positions at Richmond and Forest Park.

Qualifications

• Degree in Physical Therapy from an accredited program.
• 0-1 years of experience in physical therapy is acceptable.
• Must have and maintain a current PT license; CPR certification preferred.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises and modalities.
• Educate patients and families on exercise techniques and body mechanics.
• Maintain accurate documentation of patient progress and outcomes.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
