Job description

As a PRN Weekend Physical Therapist (WOW) at LCMC Health, no two days and no two patients are alike. You champion multiple patients as they begin their physical rehabilitation journey, whether it’s a neurological issue or a physical trauma. You encourage, comfort, and prepare patients for therapy while interacting with physicians, case managers, nurses, and families as part of a team‑care approach. You celebrate ordinary progress, recognizing that even a few steps can be an extraordinary milestone. While you focus on your patient’s extraordinary achievements, we provide the resources that amplify your impact and help you grow your expertise. Your experience, knowledge, skills, empathy, compassion, and personality all contribute to the quality of care you deliver. We look forward to learning what you bring to this critical rehabilitation role.

Responsibilities
  • Take the lead in prioritizing, coordinating, and delegating all physical rehabilitation care touchpoints for your patients.
  • Interact with the rehabilitation team to coordinate goals and improve quality of care.
  • Ensure patient physical therapy care and activities are consistent with the medical plan of care.
  • Maintain and advocate a safe environment for everyone.
  • Monitor and engage with patients, families, and appropriate care team members for teaching opportunities, patient condition changes, or concerns.
  • Ensure continuity of care, efficient throughput, consistent communication, and an exceptional experience for each patient.
  • Assist others without being asked and provide guidance to new and experienced personnel, including oversight for ancillary personnel.
  • Reassess and revise goals and treatment plans as indicated by the patient’s status, program requirements, and CMS rules and regulations.
  • Evaluate patient progress toward expected outcomes and document results of subsequent assessments, evaluations of treatment, and interventions.
  • Maintain appropriate data and contribute to departmental quality management activities, providing recommendations for systematic and programmatic improvements based on data results.
