Job summary

The Christ Hospital Home Health Care is seeking a dedicated Physical Therapist to provide skilled services as ordered by the attending physician. You will educate patients and their families, ensure patient safety, and maintain compliant documentation.

The role requires performing OASIS assessments and developing individualized plans of care. You will supervise PTAs and aides, communicate care plans, and participate in agency training.

Qualifications

• Minimum one-year experience as a PT in an acute care setting.
• Home care experience preferred.
• Must work 1 dedicated weekend day per week.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

• Patience is a virtue when working with patients, families, physicians, and coworkers.
• Attention to detail is critical, as is being observant and following directions.

Responsibilities

• Provide skilled physical therapy as ordered by the attending physician.
• Observe, record, and report patient changes to the physician and staff.
• Perform OASIS assessments to develop an individualized plan of care.
• Treat patients to relieve pain and restore function using physical means.
• Help decrease rehospitalizations by front-loading visits for high-risk patients.
• Counsel patient and family on disease/injury management.
• Instruct patient on proper use of equipment (wheelchairs, braces, walkers, etc.).
• Document all aspects of visits within 24 hours.
• Oversee and supervise therapy services provided by PTAs and aides.
• Communicate supervision results to PTAs and aides.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Follow agency policies and schedule changes.
• Attend in-service training and mandatory meetings.
• Other duties as assigned.

Work Schedule: Tuesday Wed and SAT 815-5, on call every 8th -9th weekend

SUMMARY The Physical Therapist (PT) provides comprehensive skilled services as ordered by the attending physician. The PT educates patients and their family members and ensures the safety of the patient. The PT is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.

KEY RESPONSIBILITIES

• Provide skilled physical therapy as ordered by the attending physician.
• Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
• Perform OASIS assessments to develop an individualized plan of care and makes adjustments as needs change.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as, exercise, massage, heat, water, etc.
• Helps decrease re-hospitalizations by front loading visits for high-risk patients and teaching the patient on preventative measures and making good decisions.
• Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
• Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
• Documents all aspects of subsequent, discharge, eval/recert/resumption of care visits within 24 hours of visits.
• Oversee and supervise therapy services of patient provided by PTAs and when appropriate, home health aides.
• Communicate with the PTAs and home health aides on supervisory visit results.
• Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
• Operations: Attend in-service training and mandatory agency meetings.
• Other duties as assigned.
