Cayuga Health System is seeking a Physical Therapist for the Visiting Nurse Service in Ithaca. Under the guidance of Rehabilitation Services, you will provide PT to patients at home, following physician orders and NYS standards.

The role requires NYS licensure, graduation from an accredited PT program, and at least two years of supervised clinical experience. A NYS driver's license and reliable transportation are essential.

• Evaluates functional abilities and disabilities, develops individualized treatment plans, and provides physical therapy interventions based on physician orders.
• Completes timely evaluations, re-evaluations, progress notes, discharge summaries, and OASIS assessments in accordance with agency policies.
• Collaborates with physicians and the clinical team, assesses the need for additional services, and contributes to the overall plan of care.
• Educates patients and families on therapy and participates in staff instruction and supervision.

The physical therapist (PT) works within the scope of practice as defined by the NYS Education Department for physical therapists.
