Job summary

Piedmont Augusta Hospital is seeking a Physical Therapist to deliver Home Health services. You will assess patients, determine functional status and mobility needs, and help create and implement personalized treatment plans to improve well-being and independence.

You’ll monitor progress, adjust interventions as needed, and collaborate with the care team to achieve optimal patient outcomes. A $5,000 sign-on bonus is available and a comprehensive benefits package supports you and your family.

Qualifications

• Bachelor's degree in a health-related field such as biology, exercise science, or kinesiology.
• Graduate of an accredited CAPTE-accredited program.

Responsibilities

• Assess and evaluate patient care needs related to mobility and functional status in home health.
• Develop and implement treatment plans to improve patient well-being and endurance.
• Evaluate progress and adjust interventions as needed.

The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient‘s well-being, and evaluates the patient‘s progress.

Qualifications

Education

• Bachelors Degree in a health-related field like biology, exercise science, or kinesiology Required and
• Graduate of an accredited program from a CAPTE-accredited school Required

Licenses and Certifications

• PT - Physical Therapist Upon Hire Required
• CPR - Cardiac Pulmonary Resuscitation Upon Hire Required
• DL NUMBER - Driver License, Valid and in State Must have a current driver‘s license and vehicle insurance, and access to a dependable vehicle Upon Hire Required
