Job summary

SSM Health St. Clare Hospital - Baraboo in Wisconsin is seeking a licensed Physical Therapist to evaluate and treat movement dysfunction and pain through prescribed therapy programs.

You will perform initial and ongoing assessments, establish and revise plans of care, provide education, and contribute to discharge planning and home instructions. This role includes supervision of assistants and ensuring proper documentation.

Qualifications

• Bachelor's, Master's or Doctorate in Physical Therapy or related field for licensure.
• Current Physical Therapist license in the state of practice.
• No experience required.

Responsibilities

• Performs initial and ongoing assessments of patient condition.
• Establishes, revises and evaluates a plan of care tailored to problems identified.
• Performs therapy interventions using standard PT techniques.
• Provides education to patient/family and staff as needed.
• Participates in discharge planning and home instruction.
• Oversees PT assistants, students and support personnel.
• Maintains documentation detailing response to therapy and interventions.
• Maintains safe practices and performs other duties as assigned.
