Job summary

SSM Health in Wisconsin is seeking a licensed Physical Therapist to evaluate and treat movement dysfunction and pain through medically prescribed therapy programs. The role includes establishing and adjusting plans of care, educating patients and families, and coordinating discharge planning to ensure continuity of care.

Responsibilities include supervising therapy assistants, documenting patient progress, and participating in home modifications and follow-up care as needed.

Qualifications

• Bachelor's, Master's or Doctorate degree in Physical Therapy or equivalent as required to licensure in state of practice.
• No experience required.

Responsibilities

• Performs initial and ongoing assessments of patient's condition.
• Establishes, revises and evaluates a plan of care involving patient/family.
• Performs therapy interventions using standard physical therapy techniques.
• Provides education to patient/family/staff.
• Coordinates discharge planning and home instructions with patients and caregivers.
