Job summary

Qualifications

• Bachelor's, Master's or Doctorate in Physical Therapy required to obtain licensure.
• No experience required.
• Licensed Physical Therapist in Wisconsin required.

Responsibilities

• Performs initial and ongoing assessments of patient's condition.
• Establishes, revises and evaluates a plan of care with patient/family.
• Performs therapy interventions per patient condition.
• Provides education to patient/family/staff on care needs.
• Discharge planning and follow-up care coordination with patients and caregivers.
• Oversees activities of PT assistants, students and support staff.
• Maintains documentation detailing patient response to therapy and need for skilled intervention.
